Alive 2009 trip info & sign-up
How to sign-up for Alive 2009 trip
1) Fill out and submit Alive Trip Online Registration
Form at right.
2) Download the Youth Ministry Permission & Medical
Release Form PDF. This must be filled out completely
and given to Pastor Ron by May 25.
3) Registration fee of $105.00 per person needs to be
given to Pastor Ron by May 25. Checks payable to
Living Faith Christian Community with "Alive 2009"
written out on memo line.
